Our Services

Our APRN/ Family Nurse Practitioner at the Leeward Community College Student Health Center (SHC) strives to offer high quality healthcare for students at no additional cost as all services are covered under the student’s tuition health fee.

Services Students can Receive from the Health Center Include:

  • Any Health & Wellness Services, including (but not limited to):
    • Bladder Infections/ UTI Screening and Treatment
    • FLU/ COVID Screenings and Treatment
    • Strep Throat sScreenings and Treatment
    • Evaluation and Treatment for cough/ colds, sore throats, and other Respiratory conditions, including Asthma Exacerbations
    • Injuries, Musculoskeletal Pain, Joint sprain/ injury
    • Wound Care/ I&D of abscesses
    • Gastrointestinal complaints (GERD, Acute Gastroenteritis, Nausea/ Vomiting)
    • Rashes
    • Headaches/ Migraines
    • Eye Infections
    • Allergies
    • Ear aches 
    • Urgent Care Referrals
    • Other Medical Concerns
  • Complete Physical Exams
  • Sports Physicals
  • Women’s Health Exams
  • STI Screenings, Treatment and Prevention
  • Family Planning/ Birth Control 
  • Mental Health Screenings
  • Prescription Management for most common conditions 
  • TB Clearance
  • Immunizations/Vaccinations (but not limited to):
    • Influenza
    • MMR
    • Tdap
    • Varicella

Eligibility

  • Students enrolled at Leeward Community College and who have paid the per semester health fee

Medical Records

Every patient who receives health care services at the Leeward Community College Student Health Center has a medical record. The medical record is necessary to document the care and services provided to you and to ensure a continuity of care.

Accessing Your Medical Records

All medical records are treated in a strictly confidential manner. Your written authorization is necessary to release any information, except when required by law or a court order.

Important Notice: Effective July 1, 2026, due to a change in our health record system, how you access your records depends on the date of your visit. 

For Care Received Before July 1, 2026

University Health Services Mānoa (UHSM) will retain access to these medical records. To request a copy of your records from a visit prior to this date:

  1. Complete and sign the UHSM Consent for Release of Medical Information or Records Form (PDF) .
  2. Submit the completed form to University Health Services Mānoa.

For Care Received On or After July 1, 2026

  1. Complete the Leeward CC Consent for Release of Medical Information or Records Form (PDF)  and submit it to our office. 
  2. For questions or assistance, please contact the Leeward CC Health Center at (808) 455-0515 or visit us in Room AD-223.

Service Fees

Our primary mission is to serve the student population at Leeward Community College. The SHC is supported by two funding sources: university funds and student health feesTherefore, there are no additional fees for office visits, telemedicine appointments, health clearances, consultations, vaccinations/ immunizations, referrals, lab testing and treatments provided during office visits.

 

Health Clearance Requirements

The State of Hawaiʻi mandates that certain health requirements be met for entrance to post-secondary educational institutions. (Hawaiʻi Administrative Rules, DOH Title 11, Chapter 157)

Students, including faculty/staff enrolled as students, must meet the immunization requirements before attending the University of Hawaiʻi:

  1. Tuberculosis (TB) Clearance
  2. Measles, Mumps, Rubella (MMR)
  3. Tetanus-diphtheria-pertussis (Tdap)
  4. Varicella

All of the immunizations mentioned above are available at our clinic.
