Living Well with Diabetes, Diabetes Self-Management Program (DSMP)

ALU LIKE, Inc.'s Kupuna Program will be partnering with the Department of
Health & Leeward CC to provide the following workshop for individuals with Type 2 Diabetes or have been told that they are Prediabetic. We invite you and your friends to participate if you are interested. We do ask that you register to participate ahead of time.

Although it is a workshop being offered by the Kupuna Program, it is not for kupuna only! It is for any adult with Type 2 Diabetes or Prediabetes.

The Living Well with Diabetes workshop is tailored specifically for people with Type 2 Diabetes or Prediabetes. Workshop participants have been found to lower their HbA1c levels as well as decrease their diabetes related symptoms.

This program was developed by Stanford University's Patient Education Research Center. It is designed to complement and enhance your medical
professional's recommended treatment plans and condition management. It
does NOT replace any prescribed treatment.

Living Well with Diabetes is a 6 week workshop that shows participants how to take care of themselves and do the things necessary to manage their diabetes and related symptoms. Classes are small, allowing classmates to get to know each other and work together to achieve their health goals. Many participants make friends and keep in touch after the sessions, continuing to encourage and support each other.

People who participate in and complete the workshops feel better, are less limited by their health conditions, and often spend less time at the
doctor's. Participants are able to manage their symptoms better and communicate more easily with their doctors and loved ones.

Topics covered include: goal setting, problem solving, planning for sick
days, appropriate use of medications, healthy eating, and communicating
effectively with family, friends and your health professionals.
