Complete the fillable application form by clicking directly on the fields or using the TAB button.  When completed, print and sign.  Submit application form to the Cashier’s Office (AD-113).  There is no fee for any of these certificates, however, you must be cleared of any financial obligations to the University of Hawaii system.

If you would like to participate in the commencement ceremony and receive a diploma cover, you must instead complete the Application for Graduation form and submit a $15.00 processing fee.


Prior to submitting the application to the Cashier’s Office you may elect to schedule a graduation check meeting with a Leeward Community College counselor.  To schedule an appointment, please call 455-0233.
